For each additional form you would like converted into an App, the cost is just $50 per form.
Getting Started: It's easy to get started, and typically we will have your apps back to you in 24 hours.
Step 1: Go to www.GoCanvas.com/SUYF
Step 2: FIll out the attached above referenced form.
- Simply give your new app a name
- Let us know of any special instructions for creating your app
- Click browse and locate the digital copy of the form to be converted (accepted formats are: .PDF, .DOC, .XLS, .JPG and .PNG)
- Let us know whether you would like to share your app with our Canvas community in our Canvas Application Store.*
- Once complete click submit
Step 3: Turn around time can be as much as 5 business days, but in most cases we can have something back to you within 24 hours.
*Notes: If you agree to share your app in the Canvas Application Store, you will be helping to build our community of mobile apps and users and in turn helps us to create an environment of constant improvement for all of our subscribers. As well, If you do agree to share your app in the Canvas Application Store, Canvas will remove any reference to you or your customers within your app (unless previously agreed).

I need a mobile form set in my website which allows my customers to fill a report attaching max of 10 photos and when pressing the submit button, have a copy also sent to their email they indicated in the form. Can this be done? Prices? Regards.
